The system isn't entirely clear - the F4 want to keep some mystery in it, probably to stop people from cheating in medal-gaining. However, we know it's mainly from being active in clubs and adding good contributions to them. The madami people tagahanga your images, links, artikulo and videos, the madami likely you are to get a medal there. Answering presinto and tanong and commenting on things also helps. It is a bit walang tiyak na layunin though. And I think there have been some glitches/weird occurrences of getting medalya in clubs people haven't been that active in.
