To hold one of those titles you have to contribute many things to the club that you want the medal in.
like the bigger the pamagat you want, the madami answers, picks, questions, larawan you need to add, comment on and take part in.
They're not easy to get, particularly the yellow and red ones- you have to contribute thousands of things to the club, and you basically get them out of the blue when fanpop feel like giving you one.

Remember, this is only how I get dedicated. There are other ways too.
